  • Have telephone or face-to-face interviews with all parties involved
  • Collect documentary evidence such as file notes, emails and digital material
  • Examine all the evidence to identify any contradictions and explore further into the matter
  • Give opportunities to the parties to respond to contradictions that are inconsistent with their position
  • Evaluate all the evidence, make conclusion and comment (if asked) on any breach of office policies
  • Construct a report that lays out the allegations and our findings.
